2020 Copa Sudamericana first stage
The 2020 Copa Sudamericana first stage took place from February 4 to February 27, 2020. Forty-four teams competed in 22 two-legged ties to determine 22 spots in the second stage.
The draw was held on December 17, 2019, in Luque, Paraguay. Teams were split into two pots by region and drawn into 22 ties (E1–E22) between a Pot A team and a Pot B team. In odd-numbered ties, the Pot B team hosted the second leg; in even-numbered ties, the Pot A team hosted the second leg. This setup prevented teams from the same association from facing each other.
Each tie was played home-and-away. If the aggregate score was level, away goals decided the winner. If still tied, extra time was not played and the winner went to penalties.
The 22 winners advanced to the second stage, joining 10 teams transferred from the Copa Libertadores (two eliminated in the third stage of qualifying and eight third-placed teams from the group stage).
First legs were held February 4–6 and 11–13, 2020. Second legs were held February 18–20 and 25–27, 2020.

- Unión La Calera won on away goals after a 1–1 aggregate
