The women’s 200 metre breaststroke at the 2018 Pan Pacific Swimming Championships was held on August 12 at the Tokyo Tatsumi International Swimming Center. Sixteen swimmers from six countries competed. The defending champion was Kanako Watanabe of Japan.

The event had heats in the morning and finals in the evening. For the finals, no more than two swimmers from each country could advance to the A or B finals; if a country had no swimmer in the A final, that country could have up to three swimmers in the B final.

Micah Sumrall of the United States won the race in 2:21.88, earning the gold. Her teammate Lilly King took the silver, and Satomi Suzuki of Japan won the bronze.
