The 1991 Philadelphia Phillies season was the team’s 109th. They played in the National League East at Veterans Stadium in Philadelphia and finished with a 78–84 record, placing 3rd in the division.

This was the last season the Phillies wore the 1970–91 era uniforms; the next year they went back to the Whiz Kids look, which they still use today.

Key moments from the season:
- April 28: The Phillies recorded the first triple play in Veterans Stadium history. Tony Gwynn lined a ball to Randy Ready at second; Ready stepped on second and threw to first for the outs.
- May 6: Lenny Dykstra was badly injured in a car crash on the way home from a bachelor party. Dykstra and catcher Darren Daulton, who was with him in the car, suffered serious injuries.
- May 23: Tommy Greene pitched a no-hitter against the Montreal Expos in Montreal, a 2–0 win. Greene walked seven and struck out ten; he was starting in place of Danny Cox, who had a groin pull.
- June 14: Von Hayes broke his arm after being hit by a Tom Browning pitch. Hayes returned on September 6; he was later traded to the Angels in the off-season and said the Browning incident effectively ended his career.
- July 30–August 12: The Phillies won 13 straight games, tying the modern franchise record. They swept the Expos twice during the streak and climbed from last place to 3rd in the standings.
- October 6: David Cone of the Mets struck out 19 Phillies, tying the National League single-game strikeout record and setting a record for most strikeouts by a Mets pitcher in a game against the Phillies. The Mets won 7–0.
