Takako Fujita (藤田 多佳子) is a Japanese Paralympic swimmer who competes mainly in S5 events. She represented Japan at the 2000 Sydney and 2004 Athens Paralympic Games. She was part of Japan’s gold-medal, world-record-setting 4x50m freestyle relay teams at both Games. She also competed in the 4x400m relay (20 pts) in 2000 and 2004, and in the 4x50m medley relay (20 pts) in 2004, where she won a bronze. At the 2004 Games she swam in the 50m freestyle (finished last in her heat) and reached eighth place in both the 100m and 200m freestyle finals.
