State Route 21 (SR 21) is a two-lane highway in western Tennessee, about 39.9 miles long. It runs from the Mississippi River near Tiptonville to the Kentucky state line north of Union City, and it provides access to Reelfoot Lake State Park. The route, maintained by TDOT, has existed since 1923 and crosses Obion and Lake counties.

SR 21 begins at the Kentucky border in rural Obion County as a secondary road at KY 116 and KY 239. It heads south to a nearby interchange with SR 690 (Future I-69), then meets US 51 / SR 3 / SR 22 just north of Union City. It enters Union City, travels through downtown, and then continues south with US 45W / SR 5, passing through southern Union City and then leaving the city toward Rives.

In Troy, SR 21 briefly runs with US 51 / SR 3, then turns west into downtown Troy and intersects SR 184. It heads west again to Hornbeak and intersects SR 183, continuing toward Samburg. Near Samburg, SR 21 meets SR 22 and becomes a primary highway. The two routes run together along the shores of Reelfoot Lake, cross the Hill-Talley Bridge over the Obion River into Lake County, and pass Reelfoot Lake State Park.

The highway turns west into Tiptonville and meets SR 78. At this junction, SR 22 splits off to the north with SR 78, while SR 21 downgrades to secondary. SR 21 continues into downtown Tiptonville on Church Street and ends at the Mississippi River, where the road continues as Tiptonville Ferry Road along the river levee.
